The Man in Blue is a character sporadically seen throughout the game. Depending on "You"'s course of action, he may be met regularly or never at all, apart from the initial encounter. Each meeting with him is eerily hostile and mocking on his part, as he sneers at "You"'s attempts to comprehend or ignore his presence, confronting him with his beastly outlook and uncaring philosophy on life.

The first glimpse of him is spotted in the at the end of the introductory sequence as "You” turns on his flashlight, mid-laugh with a bloody wound on his chest.

In all of his appearances, he is only known by label names such as "Seated Figure" or simply "???", although "You" refers to him as the "Man in Blue" in his diary and with conversations with either the Cat Plush or the Cat.

In the dream, "You" will be seated next to the Man in Blue, both lounging in arm chairs stood atop an empty stage. The Man in Blue will engage in bizarre conversation with "You", uncomfortably trying to make "You" not only acknowledge his actuality, but "You"'s familiarity with and reliance on the Man in Blue. He openly chastises "You" for what he perceives as naivete and will openly praise "You" if you respond aggressively towards his line of questioning, occasionally accompanied by the prompt for "You" to start "act[ing] like a man". Should "You" be running low on clips, he will supply "You" with exactly 21 bullets and can be seen as a last resort to get more ammunition.

Like the Man Who Wears a Box, the Man in Blue may only be summoned intentionally, safe for an instance where "You" completely runs out of ammo during a fight. "You" will panic and black out, triggering a Blue Dream after which the Man in Blue provides "You" with more ammo and teleport you back to bed, as if nothing had happened. This will not reset or start a new day.

When "You" enters the last building on 33rd Street, he enters the Blue Room is faced with The Man in Blue, who will ridicule him and demand he admit their acquaintance. He continuously snaps his fingers, causing a wave of static to move over the screen, each time changing the scenery slightly, first teleporting to the other side of the room and ask whether "You" knows him. If you say, that you know the Man in Blue, which negatively impacts you mental health, Chie will be dancing with him. Otherwise, a Girl in a Blue Dress will appear. Both will claim to be fine and that "You" doesn't need to worry. Depending on who is present, Chie will continue to dance while the Girl in a Blue Dress suddenly makes the Man in Blue disappear and tells "You" to move on. "You" then finds himself standing in the building's real ruined interior, with no trace of the Blue Room. There is however a chance that upon exiting the Ruined Building, a strange sphere resembling a face will quickly pass by and look out the window before vanishing. The significance of this, if any, is unknown.

Should "You" get any of the bad endings, the Man in Blue will appear to "You", to further taunt and gloat him into giving into his urges, which "You" will inevitably succumb to by invariably conceding his existence by shooting him in a rage, in an attempt to silence the Man in Blue. "You" either falls into insanity or is implied to kill himself in a maddened state, shown when his face flickers and switches to the visage of the Man in Blue, with the bullet wound vanishing from the latter and reappearing on "You". The Man in Blue jeeringly bellows and revels in his triumph over him, either cruelly admonishing him for thinking he could get rid of him so easily or contemptuously deriding him for his ignorance and feeble state of mind; the Man in Blue then replaces "You" in the end sequence, sat on the field next to "her".

The Man in Blue often exhibits an aggressive demeanor, and seems to stand in direct polarity to the "Man Who Wears a Box". This is displayed overtly in their different behaviors, and subtly by the fact that they provide two very different items: While the Man in Blue provides ammo for your handgun, which in turn promotes a violent and destructive play-style, the "Man Who Wears a Box" provides batteries and food, which promotes a peaceful and pacifist play-style.

According to Jasper Byrne, one interpretation of the Man in Blue is that the he represents the "Nature" part of the Nature VS Nurture debate. In that sense, the Man in Blue would represent that "You" is inherently violent and that his innate nature may override his choices and environment. This is supported by his nearly identical appearance, aside from age and clothing, to "You", something he himself makes note of ("Look past the age...") and attempts to face "You" with.

It is likely The Man in Blue is called 'Draco', as many notes that talk about killing the Monsters and violence are signed off by a man named 'Draco'. This is in reference to the historical Draco, a Greek lawmaker known for his unforgiving and severe punishments for even minor misdeeds.
